AGE – PROOFS

(Cir: U&R/138 dated 25/06/2016, NB & R/150 dated 25/11/2016, NB & R/197 dated 30/01/2019)

Standard Age Proofs:-
Certified extract from Municipal or other records made at the time of Birth.
Certificate of Baptism or certified extract from Family Bible if it contains age or date ofbirth.
School or college certificate or authenticated extract from the school or college records if the date of birth is statedtherein.
Certificate extract from service Register OR identity cards (provided date of birth is mentioned therein) in caseof:
Government and quasi governmentundertakings
Reputed commercial institution and industrial undertakings to their employees.
Provided conclusive evidence of age was produced at the time of recruitment of the employee.
Identity cards issued by the Defence Department to DefencePersonnel.
Marriage certificate in the case of Roman Catholics issued by Roman Catholic Church.
Domicile certificate in which the date of birth stated was proved on the strength of the school certificate or birthcertificate.
Passport (in case of illiterate proponents also accepted without any restrictions on maturity age/ Premium ceasing age, Cir: Actl/1992 dt. 21/02/2005)
Horoscopes maintained by a Hindu Family in a Bahi or Family horoscope provided Manager (NB) is satisfied with its reliability andoriginality.
PAN Card issued by Income TaxDepartment.
Driving License issued byRTO.
Aadhar Card with Full Date of Birth with followingconditions:
First 8 Digits to be Masked with Blackink
Following declaration by Proposer to be counter signed by Agent/DO: “I hereby voluntarily submit physical copy of my Aaadhar/e-Aaadhar to LIC of India for the purpose of AgeProof”

Non-Standard Age Proofs( NSAP):
Following proofs of age are considered as non-standard:
E.S.I.S.Card.
Marriage Certificate in the case ofMuslims
Service Record where age is not verified at time of entry into service or where only year of birth ismentioned.
Horoscope other than that maintained by a Hindu Family in a Bahi or family horoscopebook.
Election (Voter) Identity Card, if date of issue of card isavailable.
Certificate issued by villagepanchayat
Elder’s Declaration (Form No.3261)
Self-Declaration (Form No. 5220Stamped)
Self-Declaration (Form No. 5096 -Unstamped)
Ration Card issued originally, clearly mentioning date of issue. ( Ration card without date of issue or not legible or altered or duplicate/renewed ration card will not beaccepted)
Aadhar Card with only year of Birth with followingconditions:
First 8 Digits to be Masked with Blackink
Following declaration by Proposer to be counter signed by Agent/DO: “I hereby voluntarily submit physical copy of my Aaadhar/e-Aaadhar to LIC of India for the purpose of AgeProof”
Any other proof not classified as standard asabove.

Restrictions for NSAP:
Age proof Extra equivalent to Class I Extra, subject to a minimum ofRs.
1.50 per thousand will be charged.
Plans involving term assurance element and Term Rider will not be allowed.
Maximum age at entry will not exceed 50 yearsnbd.
Maximum Policy term will be restricted to 25years.
Maximum maturity age will be restricted to 65years.
The date of birth may be taken asfollows:
If the date of birth is stated, then the same may be taken as itis.
If the month alone of birth is stated, then 15th of the month stated may be taken as the date ofbirth.
If the year alone of birth is stated, then First July of the year stated may be taken as the date ofbirth.

If the age/ date of birth in the two types of non-standard age proofs differ, then higher of the two ages should beadmitted.
Form No 3260 is required for allNSAPs.

Maximum SUC under Non-Standard Age Proofs:

elf-Attestation of photo copy by Proposer/Life assured is must in all cases, irrespective of attestation by agent/development officer/gazetted officer.
